Supportive housing services are available to help clients secure and maintain safe and independent housing. Our client-centered services offer case management and guidance to help clients find affordable and secure housing within the community. Comprehensive Community Support Services (CCSS)
A Community Support Worker (CSW) provides individuals and families by providing them case management and resources that promote their physical and emotional well-being. CSW meets with clients in community or in their homes. The CSW identifies the client's strengths and natural resources that might hinder their ability to live independently.
Peer Support Services
Peer Support Services are provided by individuals who have gone through life experiences like those of the people they are assisting. A Certified Peer Support Worker is in recovery from mental health and/or substance use conditions and has completed a training course and passed a certification exam. Certified Peer Support Workers utilize their experience to inspire hope and empower others.
Family Peer Support workers provide coaching to families on whole health wellness to teach effective relationship skills and improve interactions between family members.
CareLink NM
CareLink NM is a group of healthcare professionals and service providers collaborating to ensure you receive the appropriate care and services necessary to maintain your health. When you enroll in CareLink NM, a care coordinator will work with you in your home to establish a care plan. Your care plan will serve as a guide to help you set goals while in CareLink NM. The benefits of CareLink NM include managing your physical and behavioral health services, social services (such as food, benefits, and transportation), and other community programs that will support and assist you and your family. CareLink NM focuses on your choices, strengths, good health, and overall well-being.
We also provide personalized education for chronic conditions, immunizations, and screening, as well as support for social networks. Our services include substance abuse prevention, smoking cessation, nutritional counseling, obesity reduction and prevention, and physical activity.
High-Fidelity Wraparound
High-Fidelity Wraparound Program works with families and aim to make their existing services more effective. It is a solution-focused approach that is driven by youth and families. The program utilizes the strengths of a child and their family to create a plan that is tailored to their unique needs and goals. The Wraparound plan is designed to help the family reach their desired outcomes by placing them in the driver’s seat. It encourages the development of natural support and helps identify new systems that may be needed. In this program, families are given the power to determine what is ultimately best for them.
Community-Based Prevention Intervention and Reunification (CBPIR)
Our Community-Based Prevention Services aim to improve parent-child interactions, promote healthy child development, and educate parents on how to meet their children's safety needs. We provide Reunification Services for families and children reuniting after being in foster care, Family Prevention Services to prevent child abuse, and Intensive Family Support to assist families in crisis. We also offer parenting classes, education on domestic violence, support for women's empowerment, and connect families with important services to meet their medical and physical needs. Our ultimate objective is to help families with their children, provide additional support, and connect them with community providers.
Juvenile Community Care Program
The Juvenile Community Care (JCC) program is a unique way of working with young people who have been adjudicated. The program adopts a team approach, including the client, their family, staff from local public schools, Juvenile Probation Officers, and other significant individuals in the client's life. Each juvenile will take part in Education Development, Job Development, Intensive Supervision, Case Management, Individual and Family Counseling, Substance Abuse Screening and Counseling, Community Service and Victim Restitution, Life Social Skills, Volunteer and Mentoring.
Home Visiting Program
Home Visitors provide educational services, child developmental guidance, and local community resource information to expectant mothers and parents and caregivers with children under five, including adoptive and teen parents. A Home Visitor will come to your home, listen to your concerns, and provide support. They can also answer questions about your baby's development and connect you with community resources. The benefits of home visiting include support for the well-being of your family and discovering ways to support your baby's development through your relationship with them.
